Gifts in Memory

Memorial gifts are a thoughtful and very special way to honor the life and memory of a beloved relative, friend, community leader or loved-one while contributing to the care and programs at our medical centers and in your community. 

An acknowledgment card will be sent to the family of the memorialized individual without mentioning the amount of the gift.

Upon the passing of a loved one, some families choose to designate the Foundation as the recipient of memorial gifts made in the deceased individuals’ name. We are touched by these heartfelt gifts and are happy to work with you to designate a fund or program that holds a special significance to your family.

Depending on the family's wishes, we can establish a memorial page on our website from which visitors can make donations while learning more about the individual being memorialized.

How to Donate

Please donate online  (writing in the tribute and comments sections) or make checks payable to Dignity Health Foundation East Valley, note on your check the name of the person you are honoring, and mail to:

Random drawing will be conducted June 12th in Arizona following all laws of the State of Arizona and IRS regulations (must be 18 or older to win) and the Arizona-based winner will be  notified via e-mail and phone to arrange for digital vacation transfer with the owner. Rules and Regulations can be found here.

Important notes for Tax and IRS Rules: 

Winner shall be responsible for any and all federal, state and local income or excise taxes, fees, assessments and like charges associated with the prize. No cash prizes offered. The IRS has taken the position that amounts paid for chances to participate in raffles, lotteries or similar programs are not gifts and, therefore, the price of the entry does not qualify as a deductible charitable contribution.
