Google Form: Fill out the Employee Giving Google Form and we'll submit your payroll request for you.

Direct Payroll Deduction: Log in to Employee Self Serve (ESS) to set up a one-time or recurring gift using payroll or PTO.
- Instructions:
- Visit ESS, and log in with your Dignity Health Legacy Employee ID Number and Password
- Navigate to "Bookmarks" → "Employee Giving" → "Add Donation"
- Confirm your details
- Credit Card: Make a one-time donation online via the "Donation form" below.
- Brochure: Download, fill out the campaign form, and drop it off or mail it to the Foundation office.
